Dismissal plans

Every inpatient admitted for acute or rehabilitation treatment has a legal right to an individual dismissal plan. The goal is to make sure that every patient recieves continous and customised care at home after leaving the hospital or rehbilitation centre.

As soon as you are admitted at our hospital we will start to record and calculate the care and support you will be expected to need after your stay at the hospital. We will help you apply for further treatment, rehabilitation, medical aides or the correct level of care. Furthermore we will support you to organise domestic, hospital or short-term care. Should it be necessary, we will prescribe medication to tide you over  until you can visit your local doctor or issue you with a notice of incapacity for work.

Your consent is needed for this process, as it enables us to pass on the necessary personal information to your health insurance or other service providers (e.g. health care supply store, nursing services). We will discuss all measures of the dismissal plan with you. We can also involve family members and relatives in the plans, if you wish.

You will be given a notice with all important information regarding the dismissal plan on admittance to the hospital. Furthermore you will be asked to fill in and sign a declaration of consent. Should you have any questions to the process of dismissal, please contact the doctor treating you.
